The form collects your name, email address, enquiry subject and message. Company and phone details are optional. You are asked to agree to the use of this information to respond to your enquiry.
Please do not submit medical records, prescriptions, test results or other health data through this form.
Use and recipients
This information is used to handle your enquiry and respond to you. It is sent by email to the Santé Expresse team responsible for your enquiry and processed by the technical providers needed for hosting and email delivery. It is not used to subscribe you automatically to a newsletter.
Retention and security
The form does not archive messages in the website database. Received emails are kept for as long as needed to handle the enquiry and maintain the relationship, subject to any applicable retention obligations.
A temporary hash of the IP address is used for fifteen minutes to limit abusive submissions. The hosting and email providers may also retain technical logs needed to operate and secure their services.
